Case Management

Robyn is able to provide complex rehabilitation case management including:

  • Assessing your rehabilitation and support needs
  • Working closely with you and your family – to understand your priorities and
  • Collaborating closely with your treating team, attendant care service provider and
    insurer, to ensure you can achieve the best outcomes possible and achieve your
    own goals
  • Help you develop your plan and support you to achieve your goals
  • Help you find suitable service providers in your local area
  • Provide injury and disability management advice, education and support, to you,
    your family and your service providers

Robyn is based in Armidale NSW, and is able to provide comprehensive case management and occupational therapy services to people living in the Northern NSW region, including towns and
rural properties in the Armidale, Guyra, Glen Innes, Inverell, Tenterfield, Moree, Narrabri, Uralla and Walcha areas.

Discharge Planning

Support to ensure you are able to access the treatment, therapy and support you will need at home after discharge. Robyn is able to visit your home and arrange the equipment and any minor modifications you will need to enable you to return home.

Assessment Services

Robyn is able to provide independent assessments and reports including ADL, home, workplace and functional capacity evaluations.

FIM Assessments

Robyn is an approved FIM TM (Functional Independence Measure) assessor.

Occupational Therapy Assessment and Programs

Development of programs to support a person increase their independence and community
participation in line with their own goals and aspirations where their physical, cognitive or
behavioral disability is a barrier to participation.

Programs may include:

  • Regaining independence or learning new skills
  • Leaning adaptive strategies
  • Prescription of assistive technology and minor home modifications
  • Transport training
  • Training of families and support workers

Training, Consultancy and Education

Robyn is experienced in service development, and training for disability service providers and allied health professionals in many areas of service delivery best practice.
